In that context, MM.08.01.01 requires organizations to evaluate the effectiveness of their medication management systems. This article provides a Key Definition of Medication Error (see comments section of this article). Most importantly, this article encourages movement toward high reliability in the area of medication management by showcasing the improvement efforts of a VA Medical center. A key component of their work was applying rate-based (e.g., number of errors per 1,000 doses) analysis to their data. The calculation method for this is explained in a straightforward manner and a sample data display is provided. |
