--We are in the process of making ongoing changes to our protocol for Coumadin use b.c of it being a JC safety goal. Our nursing staff feels strongly that they should not take telephone orders for making changes to Coumadin, unless it is to hold it or other urgent issues. --It is only an issue after hours/weekends. But I was wondering whether anyone has made things that strict with Coumadin and prohibited verbal orders for making changes to Coumadin dosages.

I have been consulting in hospital programs and have not been in a hospital that has prohibited coumadin telephone orders. If hospitls follow the read it back policy and /or have two nurses listen to the order then it should not be a problem. annemenz@consolidated.net

My experience is similar to that of Dr. Menz. I consult to several state hospital systems and have not run across the concern expressed by your nurses.
