Medication administration timeframe

Has anyone received an RFI for having a timeframe of medication administration longer than 30 minutes? If so, how did you respond?

We received a consultative finding - with the EMR it is next to impossible to do this in 30" - we use 30" on either side of the time. No happy about it but it is more realistic.

We also are using a 30 minute before and after time frame for medication. Should we anticipate any problems with this time frame do you think?

An update - From 3 TJC and 2 CMS surveys - all of my colleagues (5 separate surveys)were cited for not being in the 15" window on either side of the order time. SO we will probably seeing more RFIs.

Sue - I was really taken aback by your experience with citations based on 15-minute windows. I checked the relevant CMS standard (Survey Procedures §482.23(c)(1)) in the most recently updated state operations manual (ref to S&C-08-18 in the SL2 CMS library). It still advises surveyors to check for 'within 30 minutes'. I am concerned that either there may be a basis for challenging this or it is a new trend that our members need to be more broadly alerted to. Would you be willing to share more information? What state system are you in and how recent were your surveys? Did any of them include the same surveyors? Thanks for whatever you can share. RAF

Our practice is 30 minutes before or 30 after and we have never been cited or had issue with this by JC or CMS.

I agree. 30 minutes (before or after the scheduled time) is what our research of the standards supported and what I generally see around the country. Except for SueO's comment above I am not aware of anyone having difficulty IF they were able to stay within 30 minutes. However, that is a big IF for many of our state hospitals. I'd be curious if anyone out there has developed a med administration approach that makes it easier to meet the 30-minute window.

Sorry that I missed your message earlier. These were survey's across the country - Maine, CA, AZ, and Iowa come to mind. I am in Michigan. We have the same problem with our EMR - Cerner with the 30" but so far was haven't had a problem and I haven't heard it come up at any of the meetings or sessions I have been involved in. Sorry I can't be of more help.
