The heart of SHCC is information, but its soul is sharing.  As such, SHCC membership is a commitment to collaborating with sister systems and facilities.  In that context we would remind you that those central office staff you chose to list need to be responsive, especially to their colleagues in other central offices.  While there has not been unreasonable demand, if one's responsibilities would overly limit availability for returning calls or email, that person should be advised  against being listed.

• Re: Primary Rep - The first name entered into the form will be considered the primary representative for your state or system and as such, the first point of contact for information related to system-level questions about your facilities. 

• The CO Directory form provides for up to 7 central office staff to be listed.  If a greater number is desired, you may submit an additional form and we will combine the listing for you

• Each listing contains 6 lines.  Please place the individual's name and position on the first line.  Use the following line for telephone numbers and then an email address as follows:
